If you are unsure of your physical therapy benefits, please contact us and we can help you find out your eligibility as best as we are able. Most insurance plans cover physical therapy but may have a co-pay or deductible applied.

If you are choosing to use Medicare Part B but are unsure if you will be covered please use the following resource:

If we do not accept your insurance, we are happy to create a “superbill” for you to submit to your insurance company for out-of-network reimbursement.

Our non-insurance rates for physical therapy services are as follows:

• Initial Evaluation & Treatment: $125 (1 hour appointment)

• Package Visits: 5 visits for $550 or 10 visits for $1000 are available for patients using out-of-network insurance. Prepayment required.
